The first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to Dr. Keiko Watanabe and our caring team. We encourage questions and do our best to always deliver quality care.

Please take a moment prior to your scheduled appointment to download ALL three of our patient forms i.e. patient Information, Health History and Financial Policy. There is no need to download the HIPPA form as this is for you to read.  We ask that you complete the forms and either mail them in or bring them with you to your first appointment so we may better assist you in a timely manner.
